TERMS & CONDITIONS OF MEMBERSHIP

We apply for membership in Credit Management Association (CMA), a California Corporation doing business in California and Nevada, and agree to maintain our membership in good standing. Our membership shall become effective when accepted and approved by CMA, and each shall be for a period of one (1) year to be renewed automatically thereafter from year to year, at the prevailing prices, including any subsequent price changes, subject to termination by either party with thirty days prior written notice.

We acknowledge receiving and having access to information describing the services available from CMA and current price schedules. We also understand that there are no refunds, including fees for unused services or membership. We understand that certain services may require service related contracts.

CMA may, in cases of non-payment, send our account to collection, and any related collection fees, attorney costs, and court costs will be added to the amount due. Additionally, CMA may report our payment history to business credit reporting agencies and/or terminate our membership according to procedures set forth in CMA’s By-laws.

Terms for membership dues require payment with application, all others due upon receipt of invoice. We hereby certify that the information provided is accurate.

We acknowledge receiving a statement of our rights regarding electronic communications from CMA  and consent to receiving all communications from CMA electronically (except where we have specifically informed CMA to the contrary), and we acknowledge that consent can be withdrawn at any time by notifying CMA in writing. The undersigned is authorized to execute this agreement.

CONSENT TO RECEIVE ELECTRONIC COMMUNICATIONS

As a member and/or officer or director of CMA, you may receive communications from CMA electronically. Such electronic communications may include those delivered by (1) electronic mail (e-mail) or facsimile telecommunication when directed to the e-mail address or facsimile number, respectively, for you on record with CMA, (2) posting on an electronic message board or network which CMA has designated for those communications, together with a separate notice to you of the posting, or (3) other means of electronic communication. Please read the following statement of your rights before consenting to this method of delivering communications to you.

APPLICATION OF CONSENT
Your consent below applies to ALL communications from CMA, including without limitation meeting and event notices, member records and other information and communications provided to you by CMA. Unless it is withdrawn as provided below, the consent will continue to apply as long as you are a CMA member.

RIGHT TO RECEIVE PAPER VERSION
In those instances when you request a paper version of a particular document, CMA will mail it to you at no cost to you, with certain exceptions for unusually long documents, in which case the specific cost will be communicated to you before mailing. To request a paper version, e-mail info@emailcma.org with your specific request or call 800-541-2622 Monday through Friday, 9 a.m. to 5 p.m. Pacific time, with your specific request.

RIGHT TO WITHDRAW
You have the right to withdraw your consent at any time at no cost to you. If you wish to withdraw your consent to receive communications electronically, you must e-mail info@emailcma.org with that specific request or call 800-541-2622 Monday through Friday, 9 a.m. to 5 p.m. Pacific time, with that specific request.
